STUDENT HEALTH | 2026-27 Requirements

The health and wellness of our students and families is a top priority, and Loyola High School is committed to promoting the physical and mental well-being of our students. It's that time for students to renew/ update your physicals, baselines and online information for the upcoming 2026-2027 school year. Detailed information can be located on the Loyola website https://www.loyolahs.edu/studentlife/student-health/.

Below is what needs to be completed before practices begin for the Fall 2026 season. You will NOT be allowed to tryout or practice for any sport at Loyola until all student health requirements are completed:

  • (1) Annual Physical exam completed by licensed MD/DO
    • All physicals, please schedule appointments now. NO EXCEPTIONS!
    • Information and the physical form are found on the Loyola website https://www.loyolahs.edu/studentlife/student-health/ under the "PHYSICALS" tab. 
    • PLEASE NOTE PHYSICALS FOR THE 2026-27 SCHOOL YEAR MUST BE DATED AFTER MAY 1, 2026. Please schedule your son's physical after May 1, 2026.
       
  • (2) Annual concussion baseline, completed at Loyola
    • Completed during summer school, please schedule your appointment now.
    • You MUST complete baselines before practice begin. NO EXCEPTIONS!
    • Please click this link and follow instructions below to complete scheduling.
    • Further information about how to complete the necessary concussion baseline requirements are located on the Loyola website https://www.loyolahs.edu/studentlife/student-health/ under the "BASELINE" tab.
       
  • (3) Online/ E-Forms completed online 
    • All forms can be completed online. All forms must be completed before practice begins. NO EXCEPTIONS!
    • Information about how to complete the online forms can be found on the Loyola website https://www.loyolahs.edu/studentlife/student-health/ under the "E-FORMS" tab.
       

Please be aware that failure to complete this process could result in your son being sent home from school until all requirements are satisfied. If you have any questions, please contact Tim Moscicki at: tmoscicki@loyolahs.edu
