The health and wellness of our students and families is a top priority, and Loyola High School is committed to promoting the physical and mental well-being of our students. We are currently updating information and directions on the Loyola Student Health Website. - Current Immunization form (required by State Law)
- Form should be sent by your student's current school with final transcripts. It is your responsibility to make sure we receive your student's immunization records. Information on immunizations can be located on the Loyola website under the "IMMUNIZATION" tab on the Student Health Page.
- Form MUST be turned in before August 1, 2025.
- Annual Physical exam completed by a licensed doctor (MD/DO).
- PLEASE NOTE PHYSICALS FOR THE 2025-26 SCHOOL YEAR MUST BE DATED AFTER MAY 1,2025. Please schedule your son's physical after May 1, 2025.
- If your student is planning on participating in the summer conditioning program and/or on a Loyola athletic team during the summer; the physical form is due by the first class meeting or practice. This year that date is Jun 16, 2025.
- Otherwise, all physicals are due by Monday, August 11.
- All information and a copy of the required physical form are found on the Student Health Page under the "PHYSICAL" tab.
- Annual concussion baseline, completed at Loyola.
- ALL students are required to complete a concussion baseline. Information about how to complete the necessary concussion baseline requirements are located on the Student Health Page under the "BASELINE" tab. Baselines are completed during the summer school period.
Please be aware that failure to complete this process could result in your son being sent home from school until all requirements are satisfied.
